Complete the Prescription
Fill in all required fields including patient details, clinical
diagnosis, foot impression notes, and any specific pressure areas or lesions that must be addressed in the fabrication process.

Send Your Order
Once your prescription form is complete, package it together with the patient’s clearly labeled foam impression and send both to Perfect Step. Because every orthotic is custom fabricated from the patient’s actual impression, we cannot begin fabrication until both the completed order form and the physical impression have been received. Upon receipt, our laboratory will confirm your order and begin the review process.
